Why do you want to do fundus screening?

Premature babies are prone to retinopathy of prematurity (ROP), which will develop into serious blinding eye diseases if not treated in time. Premature infants leave their mothers when their organs are not fully developed. At this time, the development of retinal blood vessels is not mature. After birth, due to the change of blood oxygen concentration, abnormal vascular proliferation is prone to occur. Abnormal vascular rupture will lead to retinal hemorrhage, fibrous proliferation and contraction, retinal detachment, etc., and even blindness and eyeball atrophy in severe cases. ROP must be found and treated early. If treated in time, the effect is quite good.

Which children need screening and when?

For premature infants and low birth weight infants with birth weight less than 2000g, the first screening should be conducted at 4-6 weeks after birth or at 32 weeks after gestational age correction. The screening scope of premature infants with serious diseases can be appropriately expanded. The frequency of follow-up is generally determined according to the fundus lesions: once every 2 weeks without ROP, once every 1~2 weeks when ROP is diagnosed, and the interval of ROP follow-up before the threshold should be less than 1 week. Complete vascularization of retina, static lesion, scarring, or having received treatment are the termination time of screening and follow-up for retinopathy of prematurity.

What do you think of ROP inspection results?

Diagnostic criteria divide the retina into three areas:

Zone ⅰ: the central area of the retina in the circle with the optic disc as the center and the radius of twice the distance from the optic disc to the fovea maculata.

Zone II: that annular area of retina with the optic disc as the cent and the distance from the optic disc to the serrated edge of nose as the radiu.

Zone ⅲ: the temporal semilunar retinal region.

According to the severity of ROP lesions, it is divided into five stages, and the general lesions develop from stage 1 to stage 5 (as shown below). When the disease reaches stage 4, there is little hope of recovery through surgery.

There is a dividing line between the vascular area and the avascular area around the temporal side of ROP in stage 1.

Stage 3 ROP PLUS(+) not only has crista-like protuberance, but also has additional pathological changes: tortuous dilatation of blood vessels, which is more serious than that in Figure 3.

Screening process

During hospitalization, the baby’s fundus examination will be arranged by the ward; You can go to the outpatient clinic for examination after discharge.

Outpatient service process:

1. After the attending doctor determines the need for fundus screening, he will prescribe medicine for examination.

2. After the parents pay for the medicine, they start to give the baby some mydriatic liquid medicine. Generally, it takes one hour from the beginning of the medicine to the examination of mydriasis.

3. 1~2 drops of surface anesthetic water will be given before the examination, so that the baby can be examined in a painless state.

4. During the examination, the baby was lying on the bed, and the doctor opened the eyelids with a special eyelid opener for the baby. The retina was examined with binocular indirect ophthalmoscope and wide-angle children’s fundus photography system, and recorded according to the lesion area and lesion degree.

5. The doctor will inform whether treatment is needed and/or the time for the next review according to the examination results.

6. After the baby receives the examination, it needs to use anti-inflammatory eye drops for 3 days.

In order to avoid aspiration pneumonia or even suffocation caused by excessive milk during examination, parents are requested to estimate the time and stop feeding after starting to order medicine.

You need to hold your baby’s head tightly and fix it during the examination. Please ask parents to understand and cooperate. Generally, the examination time only takes 5~10 minutes. Don’t interrupt or give up the examination because of the crying of children and the distress of adults. If the blindness caused by this happens, you will regret it for life.

Investigation Report on Prepaid Consumption of Jiangsu Consumer Protection Committee

  Prepaid consumption refers to a kind of consumption mode in which consumers enjoy preferential activities of merchants in accordance with the agreed way after paying a certain fee to the operators in advance, and purchase goods or receive services in stages, that is, consumers usually encounter "card first, recharge first and then consume". In life, prepaid consumption generally exists in beauty salons, fitness, health care, laundry, retail and other industries. On the one hand, operators provide convenience for consumers, and consumers can enjoy preferential prices from merchants; On the other hand, operators can quickly increase the number of stable customers and quickly gather funds.

  In recent years, prepaid consumer disputes have gradually increased and become a hot spot of consumer disputes. In 2018, the province’s Consumer Protection Committee system accepted a total of 130,505 complaints, including 16,976 complaints about prepaid consumption, accounting for 13.01% of the total complaints. Jiangsu(Province)In 2018, the Provincial Consumer Protection Committee supported a total of 5 consumer lawsuits, all of which were prepaid consumer disputes, accounting for 100%.Some operators have hidden traps when providing goods or services, and the way of paying first and then consuming often puts consumers in a passive position, and it is impossible to predict the consistency and consistency of providing standard services. Even some operators run away after receiving the advance payment, which leads to mass incidents and affects social stability.

  The Regulations on the Protection of Consumers’ Rights and Interests in Jiangsu Province, which came into effect on July 1, 2017, clearly stipulates the scope of prepaid cards and the setting of time limit, and gives consumers the right to regret on the 15th. However, over the past year or so, disputes over prepaid cards still occur frequently.

  In order to protect the legitimate rights and interests of consumers, such as the right to know, the right to property safety, the right to claim compensation, and promote the healthy development of prepaid consumption mode, Jiangsu Consumer Protection Committee conducted a survey on prepaid consumption in Jiangsu Province around the theme of "Credit makes consumption more secure", deeply understood the current situation of prepaid consumption, analyzed the crux of the problem and put forward reasonable suggestions.

  The prepaid cards mentioned in this survey are all single-purpose prepaid cards, which are prepaid vouchers for operators to provide goods or services in advance, including physical cards with magnetic stripe cards, chip cards and paper coupons as carriers and virtual cards with passwords, serial codes, graphics and biometric information as carriers. Divided into registered cards and bearer cards.

  (3) Survey method and sample size

  This survey adopts the combination of online questionnaire survey and offline observation survey, and obtains a total of 17,546 valid samples.

  Online: With the help of the official WeChat platform of Jiangsu Consumer Protection Committee "Jiangsu Consumer Protection Committee" and Litchi News,WeChat Official AccountOnline media such as "xici" homepage conducted a questionnaire survey on prepaid card consumption. A total of 17,476 valid samples were obtained from the online questionnaire survey.

  Offline: An investigation team composed of volunteers made an investigation on 70 samples in Jiangsu Province, covering six cities in Jiangsu Province, including Nanjing, Changzhou, Nantong, Yancheng, Yangzhou and Xuzhou, and involving six industries, including beauty salons, fitness, health care, comprehensive retail, education and training, and laundry, where prepaid consumer disputes are frequent. A total of 70 valid samples were obtained from offline field experience, including 61 registered cards and 9 unregistered cards.

  Second, analysis of the survey results of prepaid card processing

  (1) Investigation on the basic situation of consumers: 85% of consumers have done prepaid cards, and the most involved fields are beauty salons, comprehensive retail and fitness industries.The online questionnaire survey shows that the proportion of male and female consumers participating in the survey is 60.8% and 39.2% respectively. 7.2% of consumers are under 18, 74.2% are between 18 and 40 years old, 17.9% are between 41 and 65 years old, and only 0.7% of consumers are over 65 years old. From the perspective of age distribution, the consumer groups are mainly young and middle-aged.

  Among online participants, 70.2% of consumers have handled it, 14.6% of consumers often handle it, 15.2% of consumers have never handled it, and nearly 85% of consumers have handled prepaid cards.

  Among them, 41.3% of consumers apply for a card because of the strong preferential treatment of merchants, 37.8% think it is convenient to settle accounts, 13% are embarrassed to refuse the promotion of merchants, and 7.8% of consumers choose to apply for a card for other reasons.

  From the point of view of the industries selected to handle the card, beauty salons (including beauty, manicure and haircut) and comprehensive retail (including department stores, supermarkets, grocery stores and convenience stores) are the most, followed by fitness, catering and education and training.

  (two) in the field experience, more than half of the operators’ advance payment exceeds the limit.

  According to the Regulations on the Protection of Consumers’ Rights and Interests in Jiangsu Province, if an operator issues a single-purpose prepaid card (including other advance receipts), the limit of a single registered card shall not exceed 5,000 yuan, and the limit of a single bearer card shall not exceed 1,000 yuan. Among them, individual industrial and commercial households need to issue single-use prepaid cards, and the single-page limit shall not exceed 1,000 yuan.

  However, the results of this survey show that many prepaid cards issued by operators have exceeded the limit. According to the online questionnaire survey data, after July 1, 2017, 42.2% of consumers said that they had paid less than 1000 yuan, 39.4% had paid 1,000-5,000 yuan, 14.3% had paid 5,001-10,000 yuan, and 4.1% had paid more than 10,000 yuan. 18.5% of consumers spend more than 5,000 yuan on prepaid card services.

  Among the prepaid cards with a single amount exceeding that of 1000 yuan, 74.1% of consumers said that the prepaid card was a registered card, and 25.9% of consumers were unregistered cards. That is to say, 25.9% of the consumption of bearer cards has exceeded the limit.

  In the off-line physical survey, 57.1% of the samples have the situation that the amount of advance payment exceeds the limit, among which the excess situation in beauty salons, health care and education and training industries is more serious, accounting for 32.5%, 22.5% and 17.5% respectively.

  (3) When handling prepaid cards, most operators will not take the initiative to sign consumption contracts with consumers.

  According to the Regulations on the Protection of Consumers’ Rights and Interests in Jiangsu Province, when an operator provides goods or services in the form of advance payment, it shall clearly agree with consumers on the quantity and quality, price or expenses, time limit and method of performance, safety precautions and risk warning, after-sales service, civil liability and other contents.

  In the online questionnaire survey, only 38.6% of the operators signed the consumption contract voluntarily when handling prepaid cards for consumers, 24% of the consumers asked to sign the contract voluntarily, and 37.4% did not sign the contract.

  In the field survey, when handling prepaid cards, only 20% of the samples signed contracts at the initiative of the operators and consumers, and 30% of the experience samples did not sign formal contracts, but registered the personal information of consumers.remain50% of the experience samples have not signed a contract.Agreement, also did not register consumer information..

  (4) The validity period of some prepaid cards is set.

  According to the Regulations on the Protection of Consumer Rights and Interests in Jiangsu Province, the validity period of prepaid cards shall not be set.

  In the online questionnaire survey, 50.3% of consumers have set the expiration date for prepaid cards, while 35.8% have not set the expiration date.

  According to the offline survey results, 27.1% of prepaid cards have a valid period, 60% of prepaid cards have no valid period, and 12.9% of them have a valid period, but the merchants verbally promise that consumers will not have a valid period when using them.

  (5) No one in the survey publicized the total amount and usage of prepaid cards.

  In the online questionnaire survey, 63.5% of consumers said that the operators did not regularly publicize the total amount and usage of prepaid cards in their business premises, and 36.5% of consumers said that the operators regularly publicized the total amount and usage of prepaid cards in their business premises.

  However, in the offline investigation, all the samples did not disclose the total amount and use of prepaid cards in the business premises. At the request of the inspectors, some merchants’ computer background systems can check the funds of prepaid cards.

  In addition, the field experience survey results show that 50% of the samples have the problem that the business license is not publicized on the wall; After the prepaid card is recharged, 14.3% of the merchants have no recharge credentials such as invoices or receipts.;After spending the card, 58.6% of the samples have paper or electronic receipts with the balance indicated, 5.7% have paper or electronic receipts without the balance indicated, and 35.7% have no consumption vouchers.

  Third, analysis of the investigation results of prepaid card withdrawal

  In 2018, the complaint data of prepaid consumption accepted by the Jiangsu Consumer Protection Committee system showed that the top three complaints about prepaid cards were that consumers were dissatisfied and asked to return their cards, businesses ran away, and consumers returned their cards for personal reasons.

  (1) Consumers’ requests for card refund are frequently rejected.The results of online questionnaire survey show that 15.9% of consumers were rejected because the merchants did not provide goods or services as agreed and asked the merchants to refund; 13% of consumers were rejected because the merchants closed down, closed down or changed their business premises and asked for a refund; 9.6% of consumers’ requests for unreasonable refund within 15 days of card application were rejected.

  In addition, in the process of prepaid card consumption, 21.9% consumers were not informed of the precautions when they applied for the card, 18.3% thought that the goods or services were inconsistent with the publicity, and 9.2% did not encounter any consumption disputes; 8% cannot be reissued because the card is lost.

        Seven suspects involved in the case were arrested, and six others were prosecuted except one because he was under the age of criminal responsibility. Wuwei Wunan Lansheng Plasmapheresis Co., Ltd. (affiliated to Lanzhou Branch of China Biotechnology Co., Ltd., hereinafter referred to as Wunan Plasmapheresis Station) was also revoked by Gansu Provincial Health Planning Commission. All these seem to indicate that this vicious incident will come to an end.

        However, a survey in The Paper (www.thepaper.cn) found that the number of underage students who were victimized may be far more than the seven identified by the police. The time for illegally collecting plasma at the plasma collection station in Wunan involved in the case may also be longer than that announced by the police from November 2013 to May 2014. However, the serious shortage of plasma and the chaotic supervision of apheresis stations in some areas are the background that can not be ignored in rethinking this case of threatening to sell blood.

Blood-selling network inside and outside campus

        Huang Xiaotian, a 14-year-old junior high school student, has been training in the school these days. He told The Paper that when he was in the fifth grade of primary school, he found that some students in his class went to Wunan Plasma Collection Station to "sell blood". There are nearly 20 students he knows who are involved in "selling blood". In his primary school class, there are seven or eight people involved.

        Huang Xiaotian recalled that most of these students who participated in "blood donation" were "fished out" for the first time. After discovering that this was also a way to get money, they went by themselves. Usually little or no money is given when it is fished for the first time, and more and more "rewards" are obtained as the number of trips increases. However, he didn’t know whether those who "fished for people" at the intersection had any connection with Zhang Moumou and others in the above-mentioned cases. He only knew that it was easier to get money by "introducing" others to "donate blood" than by himself, "just like pyramid selling".

        Some parents and teachers also found pinholes in their arms, but they generally used "bitten by mosquitoes or vaccinated" to hide the sky. Huang Xiaotian also found that a classmate in his class still wore long sleeves and wrapped himself up even in summer. In a fight, the classmate was stripped of his clothes. After everyone saw the pinhole on his arm and asked what was going on, he confessed that he often "donated blood".

        The Paper previously reported that Chen Yuhua (pseudonym) was lured to Wunan plasmapheresis station by Zhang Moumou and others in June and July 2013, and then escaped.

        Several interviewed students told The Paper that some parents took their children to the local police station to report the case. When asking the Wunan police station for verification, Yang Binghao, the director of the institute, shirked the case on the grounds that it was under the control of the city’s criminal investigation team.

        Paradoxically, according to the report of Western Business Daily in June 2013, in 2012, health supervision institutions at all levels in Gansu Province conducted 54 illegal blood collection and blood supply inspections on blood stations and apheresis plasma stations within their jurisdiction, and no illegal blood collection and blood supply behavior was found.

        According to Wuwei police, in 2014, the Wunan plasmapheresis station involved in the case required each employee to develop 30 new plasma donors, and 300 yuan was awarded every month if the target was met.         Wuwei police did not disclose how other employees completed the task. The Paper found a QQ group named "Wunan Plasma Station". Several people in the group said that they were employees of Wunan Plasma Station, but when asked about "coercing underage blood sales" and "task indicators", everyone chose silence.

        On August 22nd, The Paper saw that there were still people in the office building of Wunan plasmapheresis station. Someone looked out warily through the curtains. People in white coats called on the stairs at the back of the building, but there was no answer at the door.

The person in charge of each plasma collection station signed the 2013 plasma collection task book.

  According to the implementation measures of the regulations on the recall management of defective automobile products, "producers shall submit a recall periodic report to the General Administration of Quality Supervision, Inspection and Quarantine every three months from the date of implementation of the recall. The online service rate of government service matters is 95%, the whole process of online service and the zero running rate are 34%, and the "one online service" has been initially realized. The ability of social management has been continuously improved, public security governance has become more accurate, high-definition video surveillance of public security has been continuously improved, and a new generation of mobile police terminals have achieved 100% access to the network. The development of digital economy has entered the fast lane. The scale of digital industrialization has achieved rapid growth. In 2020, the province’s information industry achieved a main business income of 170.3 billion yuan, and the average annual growth rate during the "Thirteenth Five-Year Plan" period exceeded 20%. In the electronic information manufacturing industry, mobile terminals and displays were made in Yunnan, and silicon wafers were mass-produced, successfully breaking through a number of rare and precious metal electronic materials technologies. In terms of software and information technology services, the first provincial-level blockchain center in China was established with blockchain as a breakthrough, and a number of application demonstrations were created in the fields of commodity traceability, public resource transactions and electronic bills. The digital transformation of the industry has been accelerated, and the application scenarios of the industry have been continuously expanded. In terms of agricultural digitalization, 105 e-commerce comprehensive demonstration counties have been built, ranking first in the country. The construction of digital society has been accelerated. Public service platforms for educational resources have been deployed in all provinces, cities, counties and districts of the province, providing more than 1.2 million teaching resources. The construction of digital cities has achieved initial results. Wuhua District of Kunming, Yuxi City, Dali City, mengzi city City, Maitreya City and wenshan city City have successively become national smart city pilots, initially realizing the digitalization of urban management and effectively promoting the innovation and development of digital cities in the province. A number of municipal governance center platforms will be built, equipped with grid information collection equipment, and a grid management system covering states, cities, counties, districts, townships and villages will be formed to effectively improve the modernization level of social governance. The construction of digital hub for South Asia and Southeast Asia Radiation Center has been steadily advanced. (2) Existing problems

Although the province’s digital development has achieved phased results, there are still some problems, such as weak digital foundation support, weak digital economy competitiveness, insufficient digital innovation and application, low level of data sharing and coordination, and insufficient digital element guarantee.

First, the supporting capacity of digital foundation is weak. Our province does not belong to the national Internet backbone direct connection point, and has not yet built a dedicated channel for Internet data. The Kunming International Communication Import and Export Bureau does not yet have Internet landing services, and its traffic convergence ability is not strong in the national network structure layout, and its hub status needs to be improved urgently. The digital divide between urban and rural areas still exists, and about 5,000 natural villages with more than 20 households in the province have not achieved 4G network coverage. The digital foundation of the government is relatively weak, the application support ability is insufficient, and a unified public core basic platform has not been established. The "vertical strength and horizontal weakness" and "scattered fragments" in the construction of government information are widespread. 39.6% of government information systems run on business private networks, and all kinds of vertical business private networks are self-contained. Most of them are closed to each other and cannot be effectively connected with the government extranet, which restricts the digital transformation of the government.

Second, the digital economy is not competitive. In 2020, the added value of the core industries of the digital economy in the province will only account for 3.85% of GDP, far below the national average of 7.80%.
